What is Medication Therapy Management?

Medication Therapy Management (MTM) is a recently added patient care benefit. It is an in-depth medication review with a pharmacist. At MTM appointments, the pharmacist reviews all medications, including prescription, over-the-counter, and herbal products. They make sure each one is the right choice and combination for you. The least possible side effects and best possible results is the goal. MTM is a time for the patient (or caregiver) to ask questions about medicines and health choices. MTM also includes contact with your doctor about problems or concerns.

Medicare has determined that it is important to get an MTM review at least once each year to make sure you:

· Understand why you are taking each medication
· Are taking the correct amount of medication at the right time of day

Who Can Benefit From Medication Therapy Management?

MTM is for everyone with multiple medical problems or who takes several regular medications. Many Medicare prescription drug plans cover this service at the pharmacy including: Humana, Optimum Health, First Health Select, and Community CareRx. Even if you or a family member do not have one of these drug plans, an MTM review is an affordable way to ensure you are taking medications properly and safely.
